Most buckle fractures heal well with no long-term complications. This is the most common type of fracture in young children. Deciding which treatment is best for your child depends on the fracture pattern, the child’s comfort and the parent’s comfort level with the treatment plan. Your child has suffered a buckle fracture of the arm and wrist. Sometimes this may include a cast or splint. The typical treatment for a buckle fracture is aimed at keeping the patient comfortable while allowing the bone to heal. These are common injuries that tend to heal quickly with low risk for complications. Buckle fractures can also occur in other bones throughout the body. These fractures are most commonly seen in the wrist and are often caused by a “FOOSH” (fall on outstretched hand) injury. Because pediatric bones are softer and more flexible than adult bones, one side of the bone may buckle (or bend) upon itself without disrupting the other side of the bone. If a person is doing better after 4 weeks, repeat X rays are not needed.A buckle fracture, also known as a torus fracture, is a very common injury for children. Removable splints result in better outcomes than casting in children with torus fractures of the distal radius. Projectional radiography is generally preferable. The fracture usually occurs in children and teens because their bones are flexible, unlike adults whose more brittle bones usually break. Non-accidental injury more commonly causes spiral (twisting) fractures but a blow on the forearm or shin could cause a greenstick fracture. For urgent concerns out of hours call 111. Twenty-six patients received treatment in a Futuro wrist splint, 24 in a plaster cast, and 29 in a double Tubigrip. True torus (buckle) fractures are stable injuries. 3 Appropriate follow-up depends on the fracture type and local circumstances and may include a fracture clinic, virtual fracture clinic, private orthopaedic clinic, general practice or a combination. Activities with a high risk of falling are risk factors. All of the fractures fulfilled the criteria of being in the distal third of the radius, of being either a greenstick or torus fracture, and of being angulated <10° without a clinical deformity. It is important to remember that wrist sprains are uncommon in children. The greenstick fracture pattern occurs as a result of bending forces. This can be a very gradual chronic pain or pain from a specific injury. Symptoms include pain in the area and can start from overuse in that specific bone. Greenstick fractures are stable fractures as a part of the bone remains intact and unbroken so this type of fracture normally causes a bend to the injured part, rather than a distinct deformity, which is problematic. As per a standard fracture, the area may be swollen and either red or bruised. ![]() As these fractures are specifically a pediatric problem, an older child will be protective of the fractured part and babies may cry inconsolably. Some clinical features of a greenstick fracture are similar to those of a standard long bone fracture – greenstick fractures normally cause pain at the injured area. ![]() The name is by analogy with green (i.e., fresh) wood which similarly breaks on the outside when bent. Greenstick fractures occur most often during infancy and childhood when bones are soft. A greenstick fracture is a fracture in a young, soft bone in which the bone bends and breaks.
